Rent to Purchase Agreement for Three Different Rent to Purchase Options
Laser-Therapy.US (wholly owned by Accurate Inspections, Inc.) is a New Jersey corporation whose principal office is located at 56 Woodland Drive, Woodland Park, New Jersey 07424. The undersigned, __________________________, (hereinafter referred to as the “Purchaser”) agrees to purchase “Laser Components” which consists of:
Option A Digital desktop controller, 635nm 5mw, 808nm 150 mw laser wand see # 2A below
Option B Digital desktop controller, 635nm 5mw, 808nm 150 mw laser wand and 2 pair laser goggles see #2B below
Option C Battery operated 5 Diode Laser Head includes power supply, single wand including THREE 635nm 5mw Class 3A and TWO 808nm 150mw Class 3B lasers (12hz) plus 2 pair laser goggles see #2C below
According to the terms of this Rent to Purchase Agreement described below:
1) Upon receipt of this Rent to Purchase Agreement completed and signed by Purchaser, Laser-Therapy.US agrees to send the selected set of Laser Components to the Purchaser.
2) Until such time as all payments are promptly received the Laser Components will remain property of Laser-Therapy.US.
A) The Purchaser agrees to pay the principal sum of thirteen hundred and fifty dollars. The first payment will be three hundred and thirty dollars followed by six equal monthly charges of one hundred and seventy dollars to the credit card listed at the end of this agreement.
B) OR C) The Purchaser agrees to pay the principal sum of fifteen hundred and twenty dollars. The first payment will be three hundred and thirty dollars followed by seven equal monthly charges of one hundred seventy dollars to the credit card listed at the end of this agreement.
3) Provided payments are made timely as set forth in this agreement, there shall be no interest or finance charges due.
4) If payments are not timely received, the unpaid principal amount shall become immediately due and bear interest from the date purchase at a rate of 18%.
5) In lieu of a timely payment, the Purchaser has the option of returning the Laser Components to Laser-Therapy.US in like new condition via traceable carrier to Laser-Therapy.US, 56 Woodland Drive, Woodland Park, NJ 07424. All payments previously made will be considered rent duly owed and paid to Laser-Therapy.US and are not refundable.
6) In the event of any dispute regarding this Rent to Purchase Agreement, the Purchaser agrees, subject only to any limitation imposed by applicable law, to pay all expenses, including reasonable attorneys' fees and legal expenses, incurred by Laser-Therapy.US. This Rent to Purchase Agreement is made under and governed by the internal laws of the State of New Jersey.
7) All components are guaranteed against defect for one year. Defects caused by misuse, abuse or mishandling are not subject to this guarantee.
8) Laser-Therapy.US supplies and custom makes cold laser components for scientific and medical research use to Licensed Medical Professionals and clinicians only. The cold laser equipment components and parts are for informational and scientific research purposes only. Laser-Therapy.US does not represent that the cold laser components referenced in this agreement will achieve specific results.
I agree to pay the total amount according to card issuer agreement. I acknowledge my credit card statement will contain the above charges from: ACCURATE INSPECTIONS, INC., Woodland Park, NJ.
