If we are not able to find your NPI number or determine if you are a Licensed Medical Professional or are a Veterinarian we will ask you to fax us a copy of your professional license or provide your NPI number in order to allow us to process and ship your order.

You may fax a signed letter to (480) 393-4489 addressed to Laser-Therapy with your name and full address indicating you understand and agree to ALL the following conditions of sale:
a) As the purchaser I will use the equipment on animals I own.
b) As the purchaser I understand the equipment is not returnable for credit or refund.
c) As the purchaser I understand the equipment is fully warranted for one year (except accidental or intentional abuse).

If you are not a Licensed Medical Professional or are NOT a Veterinarian or have bought multiple units then the equipment is absolutely NOT returnable for any refund at all. In all cases our lab quality equipment is fully warranted for one year (except accidental or intentional abuse).
